In Article 21(3), for the words from “the Department” to the end of the paragraph substitute “the following persons of the making of the application—

“(a)

the Department;

(b)

the HSC trust in whose operational area the establishment or agency in question is located; and

(c)

any HSC trust that has entered into arrangements with the establishment or agency under paragraph 13 of Schedule 3 to the the Health and Personal Social Services (Northern Ireland) Order 1991.”.
